Common mistakes

Filling out the Connecticut ED 185 form, which is essential for the application or renewal of a Five-Year Renewable Coaching Permit, requires careful attention to detail. Unfortunately, individuals often make errors that can delay the process. Recognizing and avoiding these common mistakes can streamline your application process and ensure a smoother pathway to obtaining or renewing your permit.

One of the more prevalent errors occurs in Part I: Personal Information. Applicants are instructed to print all information in blue ink and in uppercase letters. This requirement is often overlooked, leading to the submission of forms that do not comply with the stipulated guidelines. This seemingly small oversight can result in the need to resubmit the application, thereby causing delays.

In the requirements for Part II and Part III, related to the completion of required First Aid and CPR courses, respectively, a common mistake is failing to sign the First Aid or CPR card before attaching it. Additionally, some applicants attach expired cards or forget to include photocopies of both the front and back of the cards. This oversight can be particularly problematic, as it directly impacts the verification of completed certifications essential for the permit.

Another frequent error found in Part IV: High School Information and Part VI: Completion of Required Coaching Course involves the attachment of supporting documents. Applicants either attach unofficial documents or fail to include them altogether. It’s crucial to attach a copy of your high school diploma or an official high school transcript alongside the necessary course completion certificates to avoid delays in the application process.

  1. Not using blue ink and uppercase letters for Part I: Personal Information.
  2. Omitting the signature on the First Aid or CPR card before attaching it to the application.
  3. Attaching expired First Aid or CPR cards, or neglecting to include photocopies of both sides of the cards.
  4. Failing to attach a copy of the high school diploma or an official transcript when required.
  5. Overlooking the submission of official documents verifying the completion of the required coaching course.

Avoiding these mistakes can significantly improve the accuracy and acceptability of your application. Ensure that every part of the form is filled out correctly and that all required documents are attached as specified. By doing so, you can expedite the processing of your application and move closer to achieving or renewing your Five-Year Renewable Coaching Permit in Connecticut.

Documents used along the form

When applying for or renewing a Connecticut Ed 185 five-year renewable coaching permit, there are several forms and documents you might also need to use and submit to ensure your application is complete. Understanding these documents will help simplify the process.

  • First Aid Certification: A photocopy of both the front and back of your valid Standard First Aid card, or the original certificate of completion. This shows that you have the necessary first aid skills required for coaching.
  • CPR Certification: Similar to the First Aid Certification, a photocopy of both sides of your valid CPR card, or the original certificate of completion, must be included. This certification is vital for ensuring the safety of athletes.
  • High School Diploma or Equivalent: A photocopy of your high school diploma or an official transcript proves that you meet the minimum educational requirements for coaching certification.
  • Concussion and Head Injury Training Certificate: The original certificate of completion for the concussion and head injury training (Module 15) from the Connecticut Coaching Education Program is required, indicating that the coach is informed about managing concussions and head injuries.
  • Proof of Coaching Course Completion: An official transcript or certificate verifying completion of an approved coaching course is required for those without a valid Connecticut educator certificate, or a standard or permanent certificate.
  • Criminal Background Check Consent Form: This is sometimes a separate document that gives permission to run a criminal history records check, which is a requirement under the Connecticut General Statutes Section 10-221d for anyone applying for a coaching permit.
  • Child Abuse and Neglect Registry Check Consent: Applicants must also submit to a records check of the Department of Children and Families’ child abuse and neglect registry, requiring another form to consent to this check.
  • Renewal Proof of Continued Education: While not initially submitted with the application, coaches renewing their permit must demonstrate completion of at least 15 clock hours of seminars, coursework, or workshops on safe and healthful coaching practices. Documentation may be requested during an audit.

Gathering these documents and forms in advance can streamline the process of applying for or renewing your Connecticut Ed 185 coaching permit. Each document plays a crucial role in establishing your qualifications and readiness to take on the responsibilities of coaching, ensuring that all coaches meet the state's standards for safety and competence in their roles.

Similar forms

The Connecticut Ed 185 form is similar to other documents required for professional certification and licensing, particularly in fields that prioritize safety and education. These requirements ensure that individuals are adequately prepared and qualified for their roles, adhering to state regulations and standards.

The form is similar to the Application for Educator Certificate in other states, like the California Commission on Teacher Credentialing's application form. Both documents require applicants to provide personal information, educational background, details of any criminal history, and proof of completed training relevant to their field. The emphasis in both forms on background checks and educational qualifications highlights the rigorous standards set by states to maintain high levels of professionalism and integrity in education.

Another document that shares similarities is the First Aid and CPR Certification application required by many states for various professions. Similar to the sections in the Connecticut Ed 185 form that require proof of completion for first aid and CPR training, these certification applications emphasize the importance of being prepared to respond to health emergencies. This requirement ensures that individuals, particularly those working in physically demanding or high-risk environments, have the necessary skills to provide immediate care.

Lastly, the National Federation of State High School Associations (NFHS) Coaches Education Program Application bears resemblance to the Ed 185 form in its structure and intentions. Both applications mandate completion of specific courses designed to equip coaches with knowledge on safe coaching practices and understanding of child and adolescent development. The focus on continuous education and adherence to updated safety practices underscores the commitment to safeguarding participants' well-being in school sporting activities.

Dos and Don'ts

When filling out the Connecticut ED 185 form, an application for a five-year renewable coaching permit, attention to detail and compliance with requirements are key to a successful submission. Here are essential do's and don'ts to guide you through the process:

  • Do use blue ink and uppercase letters for writing out your personal information in Part I, as instructed. This ensures clarity and uniformity in your application.
  • Do attach a photocopy of both the front and back of your valid Standard First Aid and CPR cards, or the original certificates of completion, as required in Parts II and III. Remember to sign your cards before photocopying them.
  • Do include the original certificate of completion for the concussion and head injury training requirement from the Connecticut Coaching Education Program (CCEP) as detailed in Part V for initial issuance and, if applicable, for renewal after July 1, 2015, attach official verification of the refresher course completed through the CCEP.
  • Do attach a copy of your high school diploma or an official high school transcript as required in Part IV. This validates the minimum educational qualifications.
  • Do truthfully disclose any criminal convictions, dismissals for cause, or any adverse or disciplinary credential actions as required in Part I, and provide a detailed statement of explanation and official documents for each incident mentioned.
  • Don't neglect to sign the attestation in Part VIII. An original signature is mandatory to confirm the truthfulness and completeness of the information provided on the application.
  • Don't forget to check the expiration dates on your First Aid and CPR certifications. They must be valid at the time of your application for both the issuance and renewal of the coaching permit.
  • Don't submit the application without the required attachments. Incomplete applications can delay or result in the denial of your coaching permit.
  • Don't provide any false, incomplete, or misleading information on the application. This can result in the denial or revocation of your certificate(s), permit(s), or authorization(s).
  • Don't wait until the last minute to submit your renewal application. Applicants are encouraged to submit the application for renewal no sooner than six months prior to the expiration date of their current permit to allow for processing time.

Misconceptions

When it comes to the Connecticut ED 185 form, utilized for the application and renewal of a Five-Year Renewable Coaching Permit, there are several misconceptions that can create confusion. Understanding these misconceptions is crucial for applicants to ensure a smooth application process.

  • Completing the form is only necessary for new coaches. This is a common misconception. In reality, both new applicants and those seeking to renew their permits must complete the form. However, the sections required and the supporting documents differ between initial applications and renewals.

  • You need to submit the actual first aid and CPR cards. Applicants are often under the impression that they must send their original first aid and CPR cards. The form only requires a photocopy of the front and back of the first aid card or a copy of the original certificate of completion. Remember to sign the card before photocopying it for the application.

  • A high school diploma is unnecessary if you have a college degree. Despite possessing higher education qualifications, all applicants must attach a photocopy of their high school diploma or an official high school transcript. This requirement stands, regardless of further education, emphasizing the comprehensive verification process undertaken by the Connecticut State Department of Education.

  • Convictions or disciplinary actions automatically disqualify you. Many believe that answering "Yes" to questions regarding past convictions or disciplinary actions related to professional certificates or licenses means automatic disqualification. However, this is not the case. Applicants must provide a detailed explanation and accompanying official documentation for each incident, which are then reviewed on a case-by-case basis.

  • All sections of the form are required for all applicants. This is incorrect. Certain sections of the form are only necessary for specific applicants. For instance, individuals already holding a valid Connecticut educator certificate have different requirements than those without. It's important to thoroughly read the instructions to understand which parts of the application apply to your situation.

By addressing these misconceptions, applicants can better navigate the requirements and successfully apply for or renew their Five-Year Renewable Coaching Permit in Connecticut. Always refer to the most current form and guidance from the Connecticut State Department of Education to ensure compliance with the application process.

Key takeaways

When applying for or renewing a Connecticut ED 185 Five-Year Renewable Coaching Permit, applicants should take note of several key aspects to ensure a seamless and successful submission process:

  • Applicants must complete all required sections relevant to their situation, using blue ink and uppercase letters for sections that require handwritten input. Different sections apply depending on whether the applicant holds a valid Connecticut educator certificate or not.
  • First Aid and CPR certifications are essential; applicants must attach either a photocopy of the front and back of their valid CPR and Standard First Aid cards or the original certificate of completion for both. These certifications must have been completed within three years prior to the application date for initial issuance.
  • Proof of completion of a concussion and head injury training module (Module 15) from the Connecticut Coaching Education Program (CCEP) is required. After July 1, 2015, a refresher course from the CCEP also becomes necessary.
  • Applicants must demonstrate ongoing professional development for renewal by having completed at least 15 clock hours of seminars, coursework, or workshops that offer insights into safe and healthful coaching practices and child and adolescent development, as approved by the State Department of Education.
  • The application includes several attestations regarding criminal history, previous dismissals, and the status of any professional licenses or certificates. Being truthful and comprehensive in these responses is crucial, as all information is subject to verification and any discrepancies may result in the denial or revocation of the coaching permit.

It's also important for applicants to note that some parts of the application require original signatures and that the application, along with all required documentation, should be mailed to the Bureau of Educator Standards and Certification as detailed in the instructions. Applications for renewal should be submitted no sooner than six months prior to the expiration date of the current permit. The Bureau of Educator Standards and Certification can be reached for further questions or clarification via email, fax, or telephone during specified hours.
